Oregon Individual & Family Plan Application Revisions/Application Submission Requirements

Application Revision
Revisions to the Oregon Individual & Family Plan application have been approved by the state. The new revision date on the application is 3/07.

We will continue to accept the previous version of the application with the revision date of 6/06 on a case by case basis only.

The 3/07 revision of the application will be available for download on the Agent Web site by Monday, July 2, 2007.

Application Requirements
Please note that separate sections of underwritten applications and plan change forms may not be combined. That is, requests requiring underwriting must be made with Oregon Individual & Family Plan application (form #2481). Non-underwritten plan changes should be done with an Individual Plan Change Form (form #3839). Combining these two forms will result in an incomplete application and may cause delays in the request.
