| PPO Portability Rate & Benefit Changes

Regence is sending a 30-day notice of rate and contract changes to these members:
- Regence BlueCross BlueShield of Oregon (RBCBSO) Prevailing and Low Cost PPO Portability
- Regence Life & Health (RL&H) Prevailing and Low Cost PPO Portability
Rate and contract changes noted below will be effective November 1, 2007, or at the member’s paid-to date.
Contract Changes
- Newborn/Adopted Child Eligibility – When additional premium is not required to add an eligible newborn or adopted child, an application will no longer be required. However, we will require written notice of the placement in order for us to know that a child has been adopted. Proof of paternity is no longer required.
- Medical Necessity – The existing medical necessity definition has been updated and replaced.
- BlueCard® – Due to a change in the BlueCross and BlueShield Association BlueCard program, wording is changed so that reimbursement for services received outside our service area will be based upon the allowed amount information of the Blue plan in the applicable service area, even when the services are rendered by providers that do not contract with those other Blue plans.
- Duplication of Benefits – Wording is added to clarify that this provision applies whether or not the other plan as a coordination of benefits provision.
- Mental Health Parity – In accordance with the Mental Health Parity legislation passed in 2005, contract language will be updated to reflect that treatment of mental illness and chemical dependency will be subject to the same level of payment and exclusions as all other medical conditions
