Oregon and Clark County Mandate Mailings
November 19, 2007               

The following letters will be mailed to Regence BlueCross BlueShield of Oregon Individual, Portability and Clark County Conversion members regarding new state mandates that will affect their benefits.  A contract endorsement will also be included in the Oregon mailing.  Washington members will be receiving revised contracts.

Affidavit of Qualifying Domestic Partnership (Oregon only)

If an Individual or Portability plan member currently has a domestic partner of the same or opposite sex, they must complete an Affidavit of Qualifying Domestic Partnership.  In addition to the affidavit, Individual underwritten plan members must complete a health statement to add a domestic partner or dependents. If a domestic partner is currently enrolled on an Individual or Portability plan with Regence BlueCross BlueShield of Oregon, the members can request to combine their enrollment under one plan to consolidate their bill.

If a Portability Plan member currently has a domestic partner of the same or opposite sex the domestic partner is eligible to enroll by January 31, 2008.  Afterwards, only newly eligible domestic partners can apply within 30 days of the qualifying event.
