Oregon and Clark County Individual Plan Application Revisions/Application Submission Requirements

Application Revision
Revisions to the Oregon and Clark County Individual & Family Plan applications have been approved by the states. The new revision dates on the applications are 3/07 for Oregon and 8/07 for Clark County plans.
We will no longer accept the previous versions of the application. Any applications with a revision date prior to 3/07 or 8/07 respectively will not be accepted after December 31, 2007.
The 3/07 and 8/07 revisions of the Oregon and Clark County applications are currently available for download on the Agent website.
Application requirements
Please note that separate sections of underwritten applications and plan change forms may not be combined. That is, requests requiring underwriting must be made with Oregon Individual & Family Plan application (form #2481). Non-underwritten plan changes should be done with an Individual Plan Change Form (form #3839). Combining these two forms will result in an incomplete application and may cause delays in the request.
