Oregon Individual Plan Application Revisions/Application Submission Requirements

Application Revision

Due to legislative changes regarding Domestic Partnership, revisions have been made to the Oregon Individual & Family Plan Application. The revisions have been approved by the state. The new revision date on the application is 3/08. The 3/08 revisions of the Oregon applications are currently available for download on the Agent website. *Any applications with revision dates prior to 3/08 will not be accepted after July 1, 2008.

Application requirements

Please note that separate sections of underwritten applications and plan change forms may not be combined. That is, requests requiring underwriting must be made with Oregon Individual & Family Plan application (form #2481). Non-underwritten plan changes should be done with an Individual Plan Change Form (form #3839). Combining these two forms will result in an incomplete application and may cause delays in the request.
