Oregon Individual Plan Application Revisions

Application Revision

Due to the closure of the $500 Deductible on our BlueSelections Premier and Plus plans, revisions have been made to the Oregon Individual & Family Plan Application. The revisions have been approved by the state. The new revision date on the application is 4/09. The 4/09 revisions of the Oregon applications are currently available for download on the Agent website. Please replace your supply of the 3/08 application with the new 4/09 version.

  • We will continue accepting the 3/08 version of the Oregon Individual & Family Plan application on a case by case basis.
  • Regence has also updated the Oregon Individual & Family Plan Change Request Form as well. Please discontinue use of the 3/08 version of this form and begin using the 4/09 version.
  • We will no longer accept any previous versions of this form for plan changes on or after 4/1/2009.
