Oregon/Clark County Individual Plan Application Revisions

The newest revision date on the Oregon Individual plan application is 4/09. The 4/09 revision of the Oregon application is currently available for download on the Agent website. Please replace your supply of the previous applications with the new 4/09 version.

  • We will continue accepting the 3/08 version of the Oregon Individual & Family Plan application on a case by case basis until May 31, 2009. After that date, that revision date will no longer be accepted.

The newest revision date on the Clark County Individual plan application is 3/09. The 3/09 revision of the Clark County application is currently available for download on the Agent website. Please replace your supply of the previous applications with the new 3/09 version.

  • We will continue accepting the 1/08 and 10/08 version of the Clark Individual & Family Plan application on a case by case basis until May 31, 2009. After that date, those revision dates will no longer be accepted.
