Clark County Individual and Family Plan Application
- Effective July 26, 2009 please begin using the new 07/09 Clark County Individual & Family Plan Application (Available on our website July 22.) We will continue accepting the 3/09 version of the Clark County Individual & Family Plan application on a case by case basis until August 31, 2009. After that date, the 03/09 version will no longer be accepted.
- There are two major revisions:
- The exemptions to the Washington State Health Questionnaire have been updated.
- The application now includes a portability warning statement requested by the Office of the Insurance Commissioner.
Washington State Health Questionnaire
- There are currently three versions of the Standard Health Questionnaire
- Standard Health Questionnaire dated April 1, 2008, for use prior to July 26, 2009. (Available on our website now.)
- Standard Health Questionnaire dated April 1, 2008, for use as of July 26, 2009. (Available on our website July 22.)
- Determination of what Standard Health Questionnaire dated April 1, 2008 to use is based on postmark date, receipt at an office location or e-signed and submitted on-line.
- Example: If an applicant is mailing their application on July 25 and it will be postmarked that day, then they would use the Standard Health Questionnaire for use prior to July 26. If they were mailing on July 26 or after, they would use the Standard Health Questionnaire for use as of July 26.
- Standard Health Questionnaire for use with coverage that begins on or after October 1, 2009. (Available on our website by September 1, 2009.)
- Use this Standard Health Questionnaire for any applications requesting an effective date of October 1, 2009, or after.
- All three versions are currently posted to WSHIP’s website at www.wship.org.
