Clark County Individual Evolve Application Revisions/Application Submission Requirements
April 1, 2011

Application Revision:  revisions have been made to our Clark County, WA Individual Evolve Plans and approved by the state. The new revision date on the application is 1/11.  The 1/11 revisions of the application is currently available for download on the Agent website.

Any applications with revision dates prior to 1/11 will not be accepted after May 30, 2011.
