Clark County Individual Evolve Application Revisions/Application Submission Requirements
July 27, 2011

Application Revision:  revisions have been made to our Clark County, Washington Individual Evolve Plans and approved. The new revision date on the applications is 7/22/11.  The 7/22/11 revisions of the application are currently available for download on the Agent website.

Any applications with revision dates prior to 7/22/11 will not be accepted after August 31, 2011.

WSHIP Standard Health Questionnaire (SHQ) Changes:  The exceptions page (page 2) of the Standard Health Questionnaire (SHQ) for Washington State (including Clark County) has been revised. The revision was made to include an additional circumstance under which a carrier may not require applicants to take the SHQ. The new SHQ is effective for coverage beginning on or after July 22, 2011.
