Oregon Individual Evolve Application Revisions/Application Submission Requirements
August 1, 2011

Application Revision:  revisions have been made to our Oregon Individual Evolve Plans to represent recent changes in Oregon enrollment guidelines for children under the age of 19. The new revision date on the application is 9/11.  The 9/11 revisions of the application are currently available for download on the Agent website.

Any applications with revision dates prior to 9/11 will not be accepted after August 31, 2011.
