Employee Choice Request Form Available on Website

The Employee Choice Request Form is now available on the Agent and Employer websites.

  • Groups who are renewing with Employee Choice can use this form in lieu of requiring their employees to complete the Application for Enrollment/Change. 
  • New Employee Choice groups will still be required to complete the Application for Enrollment/Change.
