Explanation of Benefits (EOB ) has new content

We recently revised our Explanation of Benefits (EOB) to satisfy federal health care reform requirements that recently went into effect.  The new mandate requires all carriers and group health plans to provide information to members about their right to appeal an adverse determination of benefit payment, along with government agency contact information.
