How can Regence Case Management help your patients?

A medical illness or unexpected injury can spin your patients off course and create stress and confusion.  Regence Case Managers are experienced, registered nurses and social workers who will provide one on one support and guidance.  Each member is partnered with a designated Regence Case Manager to receive individualized service.  Together, a care plan is developed to support the Physician’s treatment plan.

In addition to our Case Managers assisting with the patient’s existing medical conditions, our ProActive team of Case Managers can help with early intervention options for medical issues that might be in the patient's future. 

How can Regence Case Managers help?

  • Support the Physician’s treatment plan with development of a care plan based on the patient’s needs, social support system, policy benefits and most importantly, input from the Physician
  • Providing additional educational materials and information search tools about the condition, illness or injury
  • Serve as a member (patient) advocate and help navigate the health care system
  • Provide assistance in coordination between all services provided by Regence

Who is appropriate for Case Management?
Case Management is a service that is available to all Regence members, from birth through the golden years, who may have complex or chronic medical condition(s) or event(s).  Case Managers can also assist members (patients) who have a potential for future medical conditions.
