Preferred Choice Sixty-Five

Preferred Choice Sixty-Five is available to eligible beneficiaries in Curry, Douglas and Tillamook counties. It provides all the benefits available from Medicare Parts A and B, and Medicare Supplemental plans. Members may select either the Preferred Choice Sixty-Five plan with basic benefits or the Preferred Choice Sixty-Five Plus plan.

Key features of Preferred Choice Sixty-Five include:

• Primary care physicians (PCPs) are managers of members' health care
• No referrals are required
• Pre-authorization is required for specific services identified on the Medicare Products Pre-Authorization List
• Members must select a PCP
• PCPs are responsible for providing routine health care, including preventive health exams and treatment for illness and injury
